The Affidavit of Home School Completion is a required form for students who have completed their education through homeschooling. This form verifies that the student has successfully finished their homeschooling program and is eligible for admission to Chipola College.

The Disciplinary Disclosure Form is required for students who have answered "Yes" to the disciplinary history question on their application to Chipola College. This form must be submitted as part of your admissions process to provide a full disclosure of any past disciplinary actions, including charges and related history.

The Previous HS or College Transcript form is required for students who need to submit their official transcripts from previously attended high schools, colleges, or vocational schools as part of the admissions process to Chipola College. This form allows you to request official transcripts directly from your previous institutions to be sent to our Admissions Office.

The Residency Declaration Form is required for all students planning to attend Chipola College. This form must be completed after your application has been submitted and is available through your student portal. By completing this form, you’ll help us determine your residency status, which can impact your tuition rates and eligibility for state-funded programs.
To verify your residency status, you will need to answer a series of questions and provide two documents that confirm your residency. Commonly accepted documents include:

Official Transcript Request
The Official Transcript Request form allows students to request an official copy of their academic transcript from Chipola College. Transcripts are typically required for transfer purposes, employment, or further education. Please note the following important details when submitting your request:

Student-Only Requests: All transcript requests must be submitted by the student. Requests made by other individuals, including family members or friends, will not be processed.

The Buckley Amendment: According to the Buckley Amendment, academic records cannot be released without the student's written consent. Therefore, all requests must be made in writing by the student and include the student’s signature.

Signature Verification: All signatures must be verified. Please attach a copy of your driver's license when submitting transcript requests by fax or mail to verify your signature.

Processing Time: Allow 2-5 working days for processing. This time frame may vary depending on the volume of requests.

Financial Obligations: Transcripts will not be processed if the student has any outstanding financial obligations to the college. The student is responsible for ensuring that all obligations are resolved. Once cleared, the student must contact the Admissions and Records Office to have the request processed.

Third-Party Transcripts: The college does not release copies of transcripts from other institutions. You will need to request those directly from the previous schools.

Transcript Pickup: If you choose to have your transcript held for pickup, it will be available for 1 week. After that time, it will be destroyed, and you will need to submit a new request.

Chipola College awards three degrees, the Bachelor’s of Science Degree, the Associate in Arts, and the Associate in Science. Certificates are provided in Workforce Development programs.
To be eligible to graduate, students must:
- Complete requirements as specified in the program of study from which the student wishes to graduate, as well as satisfy any other applicable state or college requirements.
- Satisfy all outstanding obligations, financial or otherwise, to the college.
- Complete the application by logging into your student account portal MyChipola and navigate to the admissions tab to look for the graduation application on the left navigation menu.
- This application is also available at the bottom of this section.
Final responsibility for all graduation requirements rests with the student. If in doubt about course, program, or college requirements, the student should contact Student Services or program instructor for clarification and guidance. Provided all graduation requirements are completed, degrees and certificates are posted on the last date of the term in which the student applies for graduation.
College Credit students may obtain an Application for Diploma from Admissions and Records. All applications are valid for one semester. If the student does not graduate as indicated, he/she must complete a new Application for Diploma. Students applying for more than one degree will need to submit a separate application for each degree.
Certificate Clock-hour students may obtain an Application for Workforce Development Certificate from their program instructor or the Workforce Development Office.
