Documentation Requirements

The American with Disabilities Act (ADA) stipulates that it is the sole responsibility of the student to provide documentation for the purpose of determining eligibility for services. The following clarifications are provided to assist the student and their diagnostician in determining what information is needed.

Documentation must be on official letterhead of the physician/professional
Document must be signed by and dated by the physician
Documentation must include diagnosis and recommendations for accommodations
Documentation must be no more than two years old

IEP's will not be accepted as official documentation
